This nurse-led model is transforming senior foot health by utilizing AI screening technology for Ontario’s aging population.

Making health care technology accessible, convenient, and, above all, useful for seniors, their families, and their caregivers can be a balancing act. Universal Health Hub (UHH) is a leader in health care innovation that aligns with Ontario’s Health atHome vision, offering affordable, private care for seniors where they are: at home, in a long-term care or rehabilitation facility, or in hospital.

Bridging the gap: nurse-led foot care to better serve seniors

UHH was founded by Ontario nurse Maya Nathani, who recognized the growing need for accessible, high-quality care for seniors — and how technology could bridge critical gaps in a sector challenged by workforce shortages. Today, UHH delivers nurse-led foot care, personal care, and specialized geriatric services across the GTA. It also offers education and certification programs in basic and advanced foot care to empower the next generation of nursing professionals.

An important aspect of seniors’ health is foot health. Preventative foot care means fewer injuries and hospitalizations. Studies have found that seniors with foot pain are significantly more likely to experience a fall — one of the most common causes of injury for older adults. Additionally, foot infections are a leading cause of hospitalization (especially for patients with diabetes or vascular disease), and rates are elevated in those with neglected foot care. UHH has developed a mobile app to help seniors and those who care for them to identify foot health issues and then use the same mobile app to book an appointment for a licensed foot nurse to come visit.

From compassion to coding: the Reebuk Health mobile app

The Reebuk Health mobile app “uber-izes” senior foot care nursing, connecting licensed and vetted nurses with seniors wherever they age, integrating AI-driven screening, nurse-led care, and accessible foot health services. The app guides the senior or their caregiver to select the service they require, and they’ll then be matched with nurses who can help. Users can filter nurses to find someone best suited to their needs, and view nurse bios and fees. Next, users can book an appointment for the nurse to come right to the senior’s door at a scheduled date and time that works best for the senior. The mobile app has already vetted all nurses with background checks so seniors have peace of mind. It also safely processes credit card payments when confirming the appointment.

Furthering Ontario’s shift to home-based care

Reebuk Health from UHH is advancing Ontario’s Health atHome vision by providing care where the senior is. The Health atHome program is designed to ease the process of finding and getting care at home. “At home” in this case means wherever the senior is living: in their own home, at a long-term care facility, in supportive housing, or at a health care facility.

The idea of bringing more services directly to patients and seniors also aligns with Ontario’s newly announced High-Intensity Bundled Home Care program, which aims to relieve hospital overcrowding by funding home care agencies to deliver hospital-level services in patients’ homes. This type of program is perfectly supported by technologies like the Reebuk Health app, which extends the reach of home-care teams by connecting seniors with licensed nurses.

Community-centred, supportive health care for seniors using AI-driven technology is an innovative solution for a problem that leads to falls, injuries, and hospitalizations. Nurse-led preventative foot care and treatment is now as close as an app on your phone.
